Habitat Health, a healthcare organization offering fully integrated payer and healthcare services to seniors, has secured $50 million to increase access points for high-standard care in underserved communities.
Founded in 2023 by Matthew Bennett, with the support of Town Hall Ventures and Kaiser Permanente, the organization empowers seniors on Medicaid or Medicare to remain independent in their homes and communities through the Program of All-Inclusive Care for the Elderly (“PACE”).
The company’s latest round was backed by Town Hall Ventures, Kaiser Permanente, and New Enterprise Associates.
With the new capital, Habitat plans to establish its first PACE center in California by 2025, extending its services to more seniors in historically underserved low-income earning populations.
The company has also made new appointments to its team to streamline its operations. Matthew Bennett, initially serving at Evernorth as the senior vice president of care delivery, has been appointed co-founder and CEO of Habitat Health.
Carmen A. Peralta, MD, MAS, initially chief clinical officer at Interwell Health, has been appointed chief medical officer. Geoff Price, initially co-founder and COO at Oak Street Health, has been appointed the executive chairman of Habita Health’s board of directors.
